IMPORTANT PATIENT NOTICE: Practice Closure and Transition Resources
The practice of Dr. Roderick Sanden (Advanced NeuroSpinal Care) will be closing effective Friday, December 5th, 2025.
We encourage all patients to establish contact with another physician as soon as possible to ensure continued appropriate medical care.
This letter includes a list of common primary care providers and neurological/spinal surgeons previously utilized by Advanced NeuroSpinal Care.
Medical Records Request (HIPAA Authorization)
To obtain a copy of your medical records, either for yourself or to be sent to your new provider, please complete and submit the form below. The office will remain available for 60 days to consult with your new provider to promote a smooth transition.

Processing Times and Fees
- Record Availability: Medical records are kept for 7 to 10 years.
- Review Days/Hours: Records are only reviewed on Tuesday, Wednesday, and Thursday, from 9:00 am to 5:00 pm.
- Standard Processing Time: Requests will be processed within 5–10 business days.
- Expedited Processing: Requests to process Medical Records within 48 hours will incur a $100 fee.
